Cold agglutinin disease manifests as an acute or chronic hemolytic anemia. Other cryopathic symptoms or signs may be present (eg, acrocyanoses, Raynaud syndrome, cold-associated occlusive changes). Autoantibodies that attach to red blood cells only when the temperature is below 37 °C are called cold-reacting; react at 28-31°C and sometimes at 37°C. They belong primarily to the IgM class ➢ Cooling of blood during passage through acral parts of the circulation allows CA to bind to RBC and cause agglutination.
