Main element, damage of vascular wall lead to endothelial interruption, allows contact of blood and subendothelial ECM, necessary factor for formation of thrombosis and sufficient on its own to form it, often isolated in arterial/intracardiac thrombosis. Some conditions we have no true endothelial destruction but coagulant endothelial activation cause thromboresistance (bacterial toxins effect) Causes (trauma [vascular compression/contusion], circulatory turbulence [valves/vascular junctions level], inflammation [arteritis, phlebitis, sepsis], atherosclerosis)
