1. Refusal to submit for physical examination or provision of clinical samples as required for investigation; 2. Failure or refusal to provide the required information necessary for disease surveillance or response, including contact tracing activities; 3. Failure to comply with quarantine or isolation orders or directive duly issued by a public health authority; 4. Violation of the terms of quarantine or isolation; 5. Knowingly or willfully infecting another with a contagious or communicable disease classified as notifiable or a health event of public health concern, or aids in the spreading of the same.
