These are associated with intrathecal administration of MTX and include subacute meningeal irritation, stiff neck, headache, and fever. Rarely, seizures, encephalopathy, or paraplegia occur. Long-lasting effects, such as learning disabilities, have been seen in children who received the drug by this route.
