1. leaking tube 2. wrong tube 3. specimen not labelled 4. incomplete form 5. speciment insufficient 6. specimen haemolysed (only can see after centrifuge it) 7. incorrect blood-anticoagulation ratio 8. pt's ID not properly written 9. name of test not written
