The most common side effects observed were gastrointestinal issues (nausea, vomiting, diarrhea, and abdominal pain) and headaches. Like other azoles, posaconazole can cause an elevation of the liver function tests, causing elevated serum levels of hepatic transaminases. Additionally, in patients who are receiving concomitant cyclosporine or tacrolimus for management of transplant rejection, rare cases of hemolytic uremic syndrome, thrombotic thrombocytopenic purpura, and pulmonary embolus have been reported. Concomitant use of posaconazole with ergot alkaloids, pimozide, and quinidine is contraindicated.
